If you have a temporary driving permit because you lost your driver license because you refused or failed the police breath or blood test, you’ll more than likely be served notice that your license will be suspended. 

Within 15 days of your arrest date, you must request an Administrative License Revocation or ALR hearing. If you fail to request the ALR hearing, your license will be suspended.
